If you are registering for a private lesson or prefer to mail in your registration, please see the instructions and forms below.

[/vc_column_text][/vc_column][/vc_row][vc_row][vc_column][vc_column_text]Form A – REGISTRATION FORM FOR NEW STUDENTS

  • This is the new registration form. One must be filled out annually. The year runs from July 1, 2018 to June 30, 2019.

Form B – PRIVATE LESSON FORM

  • This is the form you will fill out to take a private lesson, including music.
  • Prior to submitting forms, please contact education@steelriver.org to obtain a teacher and time slot assignment.

Form C – GROUP LESSON OR PRODUCTION & PERFORMANCE WORKSHOP FORM

  • This is the form that you will fill out for a group classes, including music, production or performance  workshops. Each individual student needs his or her own form.

Form D –  PAYMENT FORM

  • This is the form you will fill out with your payment information if you are registering for a private lesson or if you choose to register by mail.
  • If you want  payment plan, please fill out this form if you choose to pay in installments – up to three (convenience fee applies to split payments).

Scholarships or Tuition Assistance:

If you cannot afford the tuition you may be eligible for tuition assistance. Please download the Tuition Assistance Form complete and send to Steel River Playhouse for consideration.

Ukulele Rental:

You can rent a ukulele from Steel River Playhouse for your class with Russ. Please download this UKULELE Rental Form and turn it in on your first day.
